Use this form to request reimbursement for purchases already made.

Check Request

Use this form to request a check for payment of a bill/invoice or for a future purchase.

Cash Box Request

Use this form to check out a DGS Parents Club cash box for an event.

Deposit Request

Use this form when you have checks or cash to be deposited into the DGS Parents Club bank account.

IL Sales Tax Exempt Certification

Use this form to make sales tax free purchases when buying items for the DGS Parents Club.
